Activity Preferences

MySecurePractice truForm allows you to receive immediate text message or email alerts by selecting the alerts you would like to receive below.

Message frequency is based on usage. PBHS does not have a separate charge for this service; however, message and data rates may apply from your mobile carrier.

This notification setting will alert you once any user submits your online truForm: 

  1. On the My Profile page
  2. Select the truForm tab

  3. Under the Activity Preferences section:

  4. Under the drop down for select the notification option you would like:

  5. Once your changes are complete, select the Save Profile button (located in the top/right hand corner):

Default lifespan of Quick Access links

Each truForm invitation will include a one-time Quick Access link, which allows patients to access their assigned forms without needing to register an account with MySecurePractice. The Quick Access link will expire if not used within the set period of time.

  1. On the My Profile page
  2. Select the TRUFORM tab

  3. Under the drop down for select the number of days you would like as your default:

  4. Once your changes are complete, select the Save Profile button (located in the top/right hand corner):
